You are signing up to receive text messages related to your use of CitusHealth, a patient engagement portal managed by your healthcare provider. These messages will include notifications regarding your CitusHealth account, care activities, and one-time passcodes. 

You can opt out of SMS messages from 36751 for US, 24887 for Canada by texting STOP to 36751 for US, 24887 for Canada. Your opt-out request will generate one final message confirming that you have been unsubscribed. You will no longer receive SMS messages from 36751 for US, 24887 for Canada if you opt out. If you want to join again, please contact your healthcare provider or text OPT to 36751 for US, 24887 for Canada. You may not be able to opt back within 30 days of opting out.
